2) Enter Name in Customer ID (a last, first name combo will do)

If you are paying with a business credit card or FOR someone else, please enter the attendee's name under 'Customer ID'". Otherwise, we will NOT be able to know WHO is attending the conference and may think the wrong person is registering or the same person is registering twice in error.

2) Enter Name, Address, City, State & ZIP AND a phone number and email (fax is optional)

    We MUST have the one name, etc. is associated with the credit card you're using,

so please enter them on the form or we cannot guarantee your registration!!!

3) Print your receipt and save it

PLEASE NOTE: WE CAN ONLY ACCEPT VISA, MASTERCARD & DISCOVER CARDS FOR PAYMENT. WE DO NOT HAVE AUTHORITY TO ACCEPT AMERICAN EXPRESS OR OTHER CREDIT CARDS. THANK YOU.

Also, remember, VA Travel Cards are for the travel portion of your trip and can NOT be used to pay for your conference tuition fee. Check with your local VA Travel Officer for details.

VAPAA members, standard registration: $375 (description: Standard Fee(before Jun 1, 2010) for members of the Veterans Affairs Physician Assistant Association for the VAPAA CME conference July 12-16, 2010.)
(We WILL not let you enter the conference if you are not a confirmed member for this rate. You will be charged the appropriate rate plus a service charge. However, if you sign up as a member the same day as you register you can get the lower rate saving $50.)
Non-VAPAA PAs: $475 (description: Standard Registration Fee(before Jun 1, 2010) for the VAPAA CME conference July 12-16, 2010., for Physician Assistants OR other healthcare professionals who are NOT eligible as members of the VAPAA.)
PA Students: $125 (description: CME Conference Student registration (anytime) for the VAPAA CME conference July 12-16, 2010, for students enrolled in an ARC-PA accredited PA program.)
